Body Mass Index (BMI) is one of the most widely used screening tools for assessing weight status in adults. While not a direct measure of body fat, BMI provides a quick and cost-free way to identify potential weight-related health risks and is used by healthcare professionals worldwide as a starting point for health assessments.

What is BMI?

Body Mass Index (BMI) is a numerical value calculated from a person's weight and height. It was developed by Belgian mathematician Adolphe Quetelet in the 19th century and has been formally adopted by the World Health Organization (WHO) as the standard screening tool for weight classification in adults worldwide (WHO, 2000).

BMI does not directly measure body fat percentage — two people with the same BMI can have very different body compositions. A muscular athlete and a sedentary person of the same height and weight would have the same BMI, but very different body fat percentages. This is one of the primary limitations of BMI as a diagnostic tool.

Despite its limitations, BMI remains valuable because it correlates reasonably well with health risk at the population level, is inexpensive to calculate, non-invasive and quick. Medical research consistently shows that higher BMIs are associated with increased risk of type 2 diabetes, cardiovascular disease, hypertension, certain cancers, sleep apnea and osteoarthritis.

The BMI Formula

BMI is calculated using a simple formula based on weight and height:

BMI Classification Chart

The WHO classifies BMI into the following categories for adults 20 years and older:

• Below 18.5 — Underweight: Associated with nutritional deficiencies, weakened immune function, and in severe cases, eating disorders. Underweight individuals should consult a healthcare provider.

• 18.5 to 24.9 — Normal Weight: Associated with the lowest risk of weight-related health conditions. Maintaining a BMI in this range through healthy diet and regular physical activity is optimal for most adults.

• 25.0 to 29.9 — Overweight: Associated with moderately increased risk of cardiovascular disease, type 2 diabetes and joint problems. Lifestyle modifications including diet and exercise are typically recommended.

• 30.0 to 34.9 — Class I Obesity: Associated with high risk of metabolic disorders. Medical supervision and structured weight management programs are usually recommended.

• 35.0 to 39.9 — Class II Obesity: Associated with very high health risk. Medical treatment including medication or bariatric surgery may be considered.

• 40 and above — Class III (Severe) Obesity: Highest risk category. Also referred to as morbid obesity. Immediate medical intervention is typically recommended.

BMI thresholds may differ for certain ethnic groups. Research shows that people of Asian descent face increased health risks at lower BMI values (overweight = 23+, obese = 27.5+).

Limitations of BMI

While BMI is a useful population-level screening tool, it has important limitations that users should understand:

It does not distinguish between muscle and fat mass. A bodybuilder may register as "obese" despite having very low body fat due to high muscle mass.

It does not account for where fat is distributed. Abdominal (visceral) fat is far more dangerous than fat stored in the hips and thighs, and two people with the same BMI can have very different fat distribution patterns.

It may be less accurate for older adults (who tend to have more fat and less muscle), women (who naturally carry more fat than men at the same BMI), and certain ethnic groups with different body composition norms.

For a comprehensive assessment of weight-related health risk, healthcare providers use BMI alongside waist circumference, body fat percentage measurement, blood pressure, cholesterol levels and blood glucose.
